The duration of the ankle clonus correlates with the severity of serotonin toxicity.
In order to elicit the sign you should vary the amount of pressure you exert with the hand that is forcibly dorsi-flexing the foot. Too much or too little pressure doesn't allow the clonus to manifest.
Can be seen spontaneously in some patients when they are looking forward, they may complain of bobbing vision (things bobbing up and down but in focus). More commonly it is seen when you ask the patient to follow your finger. Unlike nystagmus there is no fast or slow component
